Sygma House provides personalized residential assisted living in a warm, family-style setting for adults who need daily support in Minnesota.
Why Families Choose Us
With only 5 residents, every person receives individualized attention. We are not a facility — we are a home.
Only 5 residents means your loved one is never lost in the crowd. Staff know each resident personally — their preferences, routines, and what brings them joy.
A genuine residential home in a neighborhood setting. Shared meals at the dining table, cozy common areas, and private bedrooms that residents can personalize.
Fully licensed under Minnesota Chapter 144G. We follow UDALSA standards including 144G.91 resident rights and required disclosure practices.
Trained staff assist with medication administration, coordination with prescribers, and pharmacy relationships to keep residents safe and healthy.
We work with Elderly Waiver (EW), CADI, and other MA waivers. Our team can coordinate with county case managers and MnCHOICES assessors.
Regular updates, open-door policy, and responsive staff. We believe families are partners in care, not visitors.
The trust of families is the foundation of everything we do.
Moving my mother to Sygma House was the best decision our family made. The staff remembered her favorite shows, her tea preference, everything. She felt at home within a week.
As a discharge planner, I've referred multiple clients here. The communication is excellent, the intake process is smooth, and the families consistently report high satisfaction.
I was nervous about Medicaid waiver coordination, but the team at Sygma House worked directly with our county case manager. The transition was seamless.

A licensed residential assisted living home built on the belief that every adult deserves dignity, connection, and a real home.
Sygma House was founded with a single purpose: to create the kind of assisted living home we would want for our own family members. Not a clinical facility, not a large institution — a genuine home where people are known by name, not room number.
We serve adults who need daily support but want to maintain their independence and dignity in a residential setting. With only five residents, our staff have the time and capacity to truly care for each individual.
Our home is licensed by the State of Minnesota under Chapter 144G (the Uniform Demonstrated Ability and Licensure Standards for Assisted Living Act) and we welcome residents on private pay and Medicaid waiver programs including the Elderly Waiver.
Sygma House holds an active Assisted Living Facility license issued by the Minnesota Department of Health under the UDALSA framework.
We comply with all 144G.91 resident rights including access to records, privacy, grievance procedures, and freedom from discrimination.
